Police said the man was driving his Volkswagen Golf along Cotham Road at Kew about 12.15pm on November 18.
The driver sounded his horn at an e-scooter rider, and what followed was “incredibly terrifying”, Victoria Police Senior Constable Callum Allchin said.
“The motorist has tooted his horn at the e-scooter rider to let them know he was behind them.
“The e-scooter rider has then started to verbally abuse the victim.
“It was completely unjustified, completely out of proportion, and it was violent.”
The rider hurled verbal abuse at the Volkswagen driver before turning to physical violence.
CCTV vision shows the rider kick and punch the victim’s side mirror and smash his windscreen.
He then left the scene, police say.
The attacker, who remains on the run, caused $4500 worth of damage to the vehicle.
Police have released images of a man they believe might be able to help the investigation.
The 74-year-old is the second motorist attacked at the same intersection in less than two weeks.
In another instance, a food delivery rider was confronted by three young people on a busy shopping strip.
One teenager rode off with the man’s motorbike, while another chased him down the busy shopping strip with a knife.
The teenagers made off empty handed and have not been arrested.
